Payment Policies

The Lactation Network clients:

As long as your visit has been pre-approved by The Lactation Network, you should have no extra fees.  Please contact TLN if you have billing questions. 1 (888) 510-0059

 

Self-pay (out-of-network) clients: 

  I will provide you with a superbill suitable for you to submit to your insurance. The superbill (which will also serve as a payment receipt) will be coded appropriately to the level of service provided during the visit. You agree to pay me at the time of the visit (cash or check). 

Understand that you must pay the full self-pay fee up front as a deposit.

Milk Haven Lactation ​is providing care to you and to your baby or babies; together and are all the client of Milk Haven Lactation​.  

Initial visit includes 1 week of follow up support by secure messaging, email, or text. Continued support is available upon request or by an additional appointment. 

You are responsible to verify your own lactation benefits. Milk Haven Lactation can only see that you have benefits, we cannot see if you have any special circumstances that might prevent your insurance provider from covering services. If your plan denies coverage of lactation services after the claims have been submitted, you are responsible to pay at the self-pay rate. You should refer to my plan benefits and call your insurance directly to verify lactation coverage.

Milk Haven Lactation may communicate your insurance company in reference to the services provided to me and my baby or babies.  Milk Haven Lactation may communicate with my credit card company or bank for any payment related matters.  It is your responsibility to provide accurate and current payment and insurance information. 

These policies apply to Milk Haven Lactation and its representatives.

If you use SquareUp: Payments may be made electronically using a credit card or fund transfer.  I use SquareUp to process payments. SquareUp meets the high standards of HIPAA and the banking industry for security and privacy with regard to financial transactions.  However, SquareUp may send, automatically or per your request, email or text message receipts that reveal personal health information such as the date and type of lactation visit. If you are not comfortable with this, payment may be made via cash or check instead. 

Cancellation policy: You are responsible for all charges associated with your visit. If you cancel with less than 24 hours notice, you will be charged $20. I understand that unforeseen circumstances occur. If you have an emergency or need to reschedule, please contact Brandi at Milk Haven Lactation: 815-252-6540.
